About the Role
We are seeking an experienced Care Manager to lead the clinical operations of our residential aged care home. This role offers the opportunity to mentor a dedicated clinical team, uphold aged care compliance standards, and champion high-quality, person-centred care. You will play a key leadership role in fostering a safe, compassionate environment for residents and staff alike.
Key Responsibilities
- Oversee the day-to-day clinical operations of the facility
- Lead, support and mentor clinical staff, fostering professional development
- Develop and manage tailored care plans in collaboration with families and health professionals
- Monitor compliance with Aged Care Quality Standards and clinical governance frameworks
- Manage care documentation, audits, and accreditation processes
- Engage in continuous quality improvement and risk management initiatives
- Build strong relationships with residents, families, and stakeholders
Requirements
- Registered Nurse with current AHPRA registration
- Proven experience as a Care Manager or in a similar senior clinical role
- Sound knowledge of Aged Care Quality Standards and AN-ACC funding
- Strong leadership and communication skills
- Competency in clinical care planning, evaluation, and team management
