Position Overview:
The Home Care Worker will provide essential in-home care and support services to individuals, assisting clients with personal care, domestic tasks, and social support. This role is vital to ensuring that our clients maintain their independence, dignity, and quality of life, enabling them to live comfortably in their own homes.
Key Responsibilities:
- Personal Care Support:
- Assist clients with personal hygiene tasks, such as bathing, grooming, dressing, and toileting.
- Provide mobility assistance, including help with walking, transfers, and using mobility aids.
- Domestic Support:
- Perform household tasks such as light cleaning, meal preparation, laundry, and shopping assistance.
- Ensure the home environment is safe, tidy, and hygienic.
- Social and Emotional Support:
- Offer companionship to clients, engaging in meaningful conversations and activities to promote social interaction and reduce isolation.
- Assist with outings and appointments, providing transport as needed.
- Health and Wellbeing:
- Monitor and report on clients' physical and emotional well-being, including any changes in condition to the Home Care Manager or healthcare team.
- Assist with medication reminders and health monitoring (as directed by the care plan).
- Client-Centered Care:
- Work with clients and their families to ensure that care plans are followed and provide feedback for improvements.
- Respect the preferences, cultural practices, and confidentiality of clients.
- Record Keeping:
- Maintain accurate and up-to-date client records, including progress notes, timesheets, and any changes in care requirements.
- Report any incidents, concerns, or changes in condition to the Home Care Manager immediately.
Key Selection Criteria:
- Experience:
- Previous experience in home care or personal support work (preferred but not essential).
- A passion for supporting the elderly and people with disabilities to live independently and with dignity.
- Qualifications:
- Cert III in Individual Support (Ageing, Disability, or Home and Community Care) or equivalent (preferred).
- Minimum of three (3) months, paid employment experience in a Support Worker, Care Worker, Nursing role
- First Aid and CPR certification (or willingness to obtain).
- Skills and Attributes:
- Excellent communication skills, both verbal and written.
- Ability to work independently and as part of a team.
- Strong time management and organizational skills.
- Empathy, patience, and a genuine care for others.
- Ability to adapt to clients' changing needs.
- Other Requirements:
- A valid driver’s license and reliable transport (for visiting multiple clients in the region).
- National Police Check (or willingness to undergo one).
- Working with Children Check (if applicable).
